Wow! That's the first time I have ever seen the ratchet teeth stripped like that.

So I dug a little further and I removed the plunger and heavy duty spring from the tensioner. Underneath all of that is a valve with a bearing and a spring. When I took it apart the spring that holds pressure on the bearing was broken.
